YOU ARE HEREBY NOTIFIED that a petition has been filed in the Court alleging that the above-named minor child is a Deprived Child and that, as the parent (or alleged parent) of said Child, you have been named as the Respondent, all as more fully set out in the attached petition.

YOU ARE THEREFORE ORDERED TO APPEAR at the courtroom of the Chickasaw Nation District Court, 103 E. Main, Ardmore, Oklahoma, on the 17th day of August, 2026, at 9:30 o'clock A.M. and to there remain subject to the call of the Court until discharged so that you may be advised of the allegations contained in the petition and may answer that you admit or deny the allegations of the petition.

You may seek the advice of an attorney on any matter relating to this action at your own expense, or upon application to the Court, you may be eligible for a court-appointed attorney.

FAILURE TO RESPOND TO THIS SUMMONS OR TO APPEAR AT THIS HEARING CONSTITUTES CONSENT TO THE CHILD AS A DEPRIVED CHILD AND MAY ULTIMATELY RESULT IN LOSS OF CUSTODY OF THIS CHILD OR THE TERMINATION OF PARENTAL RIGHTS TO THIS CHILD.
